card information) filled in ready to print, fill in
the form below on-line, press the submit button, then
print the page and follow the instructions at the
top of your printed form to send to us. NB All items
in red must be completed
or the form will show an error.
If
you cannot use the form, or would prefer to fill in
all details by hand, please print and complete our
text form instead.

be generated with all your details completed except for your credit card details
- Print the page
and fill in your credit card details and signature
- Post or fax
to the Embroiderers' Guild at the address shown on the page
